What cerebrolysin is
Cerebrolysin is not a single peptide - it is a mixture of small peptides and amino acids made from pig brain tissue. It is given by IV infusion, usually in courses, and is used abroad on the theory that it acts like the brain's own growth and repair signals (neurotrophic factors).
It is approved in many European and Asian countries for stroke, traumatic brain injury, and dementia - but it is not FDA-approved in the United States.
What the evidence actually shows
Vascular dementia. This is cerebrolysin's best-studied use. A randomized, placebo-controlled multicenter trial reported improved clinical outcomes[2]. But the more important document is the Cochrane review - an independent analysis of all the trials together. It found that cerebrolysin might improve cognition and function, but the trials had high risk of bias, the results were inconsistent, and any real benefit may be too small to be clinically meaningful[1].
That is a crucial distinction: "some trials looked positive" is not the same as "the evidence holds up when examined carefully." For cerebrolysin, it partly does not.
Stroke. Results are mixed. A large trial cast doubt on cerebrolysin's usefulness for most stroke patients, with possible benefit only in severe cases.
Alzheimer's disease. Some trials suggest small improvements, but again the evidence is limited.
The honest grade
Cerebrolysin sits in an interesting spot: more evidence than most peptides, but evidence that independent reviewers rate as weak.
